The plantar insole of this diabetic walker provides effective pressure relief, reduces shear forces, and allows for easier ulcer management compared to a total contact cast. Pressure relief is easily created by the removal of numerous independent shock-absorbing hexagons. The insole can accommodate various sizes, shapes and numbers of ulcers. This model reduces shear by fixing the ankle at 90 degrees and incorporating a rocker bottom sole. This diabetic walker is the easiest to apply compared to other methods for treating mild to moderate ulcers.

The smallest wrinkle, poor molding or tension deviations can initiate severe problems for the patient. Plantar ulcers generally take months to heal and a TCC needs to be changed weekly. Due to the extensive labor involved, the cost of choosing a TCC for treatment can be quite high. A total contact cast that uses a customized walking heel instead of a diabetic boot can cause instability in the patients gait. TCCs are not recommended for people with soft tissue infection.
